All students are required to have a physical exam filled out and signed by their physician. Please submit a signed physical to your school nurse within 30 days of enrollment.

• All children entering Pre-Kindergarten.
• All children entering Kindergarten if they do not already have one on file.
• All students new to New Jersey.
• Any other student who does not have a physical exam on file.

Please have your child’s physician fill out and sign the Universal Child Health Record.
